• College Undergraduate forms page is where you will find the Course Substitution Petition form, the Prerequisite Waiver Request form, and the Request for Override for Capacity, Field of Study, Department Permission form.
• Registrar's Office forms: All the forms from the USC Registrar's office, including the Independent Study form.
